Job Title: Care Coordinator
Location: Carmarthen
Salary: Up to £30,000 per annum
Working Hours: Monday - Friday, 8:00 AM to 4:00 PM
We are currently seeking a dedicated and experienced individual to join a fast growing domiciliary care organisation as a Care Coordinator in Carmarthen. As a Care Coordinator, you will play a crucial role in ensuring the effective coordination of care services to enhance the quality of life for the clients.
Key Responsibilities:
- Coordinate and schedule care services for clients in a timely and efficient manner.
- Liaise with care staff and clients to ensure service delivery meets the highest standards.
- Conduct assessments and reviews to determine the appropriate level of care required.
- Maintain accurate records of client information and care plans.
- Collaborate with healthcare professionals to provide comprehensive care solutions.
- Respond to queries and concerns from clients and their families.
Qualifications and Experience:
- Previous experience in care coordinating is essential.
- Full UK driver's license is mandatory.
- Strong organisational and communication skills.
- Ability to work independently and collaboratively in a team.
- Knowledge of care regulations and best practices.
Working Hours:
This is a full-time position with working hours from Monday to Friday, 8:00 AM to 4:00 PM.
Salary:
The salary for this position is up to £30,000 per annum, depending on experience and qualifications.
