Job Description

Role: Case Management & Intervention Lead

Salary: £32,076 - £33,9540 + Secure Unit Allowance £1,291 PA

Hours: 37 per week Monday-Friday 8am-4pm / 9am-5pm

Location: Leeds

About Us:

We are dedicated to providing compassionate care for vulnerable young people aged 10 to 18. Our residents come to us through various avenues, seeking guidance and support due to their unique circumstances. We are currently seeking a dynamic individual to fill the role of Case Management & Intervention Lead.

Position Overview:

Are you resilient, empathetic, and driven to inspire positive change in the lives of vulnerable youth? If so, you might be the perfect fit for our team. As the Case Management & Intervention Lead, you will play a pivotal role in the lives of young people under our care, guiding them towards progress, development, and transition opportunities. You'll lead a unit of eight young individuals, employing restorative and solution-focused approaches to support their complex needs and address challenging behaviours.

Responsibilities:

Lead and coordinate the case management and delivery of targeted interventions for a unit of eight young people.

Conduct high-quality assessments, develop care plans, and provide effective interventions tailored to individual needs.

Foster effective communication and collaboration with colleagues and external partner agencies to ensure integrated service delivery.

Coordinate and attend reviews for young people, ensuring timely scheduling and adherence to established protocols.

Contribute to the positive development of young people through proactive engagement and the delivery of care and intervention programs.

Maintain accurate records of each young person's progress and interventions.

Qualifications & Requirements:

NVQ/QCF in Health & Social Care (Children & Young People) Level 3 required.

Working towards QCF Assessors award preferred.

Extensive knowledge of child and adolescent development, youth justice systems, and safeguarding procedures.

Minimum of three years of direct experience working with young people displaying complex behaviours.

Strong communication skills and ability to manage difficult situations with professionalism.
