Complex Care Manager
Position: Complex Care Manager (CCM)
Location: Hybrid - Wolverhampton office and oversight for the North Region
Type: Full-time
Salary: £35000 to £40000

Role Overview:
As a Complex Care Manager, you will be responsible for organising, facilitating, and ensuring the delivery of complex home care services. You will work closely with teams of care assistants and support workers, reporting directly to the head of service. This role involves both the setup and coordination of services for new clients and the ongoing maintenance of services for existing clients.
Key Responsibilities:
- Service Coordination:
- Set up and coordinate services for new clients.
- Maintain and ensure high standards of care for existing clients.
- Client Interaction:
- Maintain constant contact with service users to ensure person-centred care.
- Liaise with clients to understand their preferences for contact and address any concerns.
- Collaboration:
- Work collaboratively with clinicians, Clinical Commissioning Groups (CCGs), and other professional bodies.
- Meet regularly with clinicians to review and ensure care packages meet defined clinical standards.
- Reporting:
- Prepare and send monthly reports to the CCG on ongoing care provided.
- Field-Based Duties:
- Provide high-quality care in the field, covering emergency shifts if needed.
- Ensure all clients receive services as designated in their care plans.
Requirements:
- Valid driving licence and own vehicle.
- At least 5 years of experience in community care.
- Ideally, experience in complex care areas such as ventilation and spinal injuries.
- Ability to work independently and remotely.
- Willingness to visit the Wolverhampton office bi-weekly for management meetings.
- Passion for supporting service users, families, and staff members.
- Proven ability to work with families and individuals facing significant life changes.
- Flexibility to share on call
