Job Description
Job Title: Complex Needs Practitioner
Salary: Up to £30,000
Location: Birmingham
I am currently working with a market leading charity service that offers support across Birmingham and Solihull for individuals dealing with substance misuse, including issues stemming from family-related substance misuse.
The role aims to engage individuals in various interventions through a person-centered approach, empowering them to reduce risks, build resilience, and achieve positive outcomes. This also involves motivating and encouraging individuals to access additional services or broader treatment options.
A key aspect of the role involves working with individuals, their families, and community networks to address holistic support needs across all areas of wellbeing. This may include addressing issues such as knife crime, family support through the 5-step method, exploitation, physical and emotional health, detoxification, rehabilitation, and other substance misuse needs, as well as providing intensive support.
- Support and build relationships with people, and their families, who have a variety of needs related to substance misuse, either their own substance use or somebody else's
- Hold a caseload of complex cases requiring a case co-ordination approach. Requiring you to complete all recording and case management administration for your caseload accurately and promptly, adhering to GDPR and Data protection policies
- Develop co-produced, outcome focused support plans that reflect each participant's personal strengths and goals. Support plans will be holistic, covering all aspects of the person's wellbeing.
- Co-ordinate interventions provided by other agencies to ensure that the right support is offered at the right time to enable the recipient to progress on their journey to recovery.
- Attend and participate in clinical case discussions, completing agreed actions
- To advocate for wider treatment options and additional interventions required.
