Homelessness Prevention Support Worker

£12.50ph

Monday-Sunday - Days & Nights

DBS on update service would be beneficial

About the role :

Work with some of the most vulnerable people to find a way forward. Aid and support individuals in getting off the streets, out of abuse, overcome skill barriers, to secure permanent accommodation, and help to equip them with the skills to live independently.

Progress and track referrals from the Local Authority into the service and outcomes for clients moving on from the service.
Delivery a high-quality off the street accommodation service for individuals rough sleeping within the Yeovil area
Complete holistic assessments for clients leading to the development of support plans which meet individual client goals.
Implement person-centred packages of support with clients.
Give clients advice, information and provide an enabling service so that they can be signposted to and access appropriate services to develop independence
Provide an effective Housing Management service for clients and identified properties.
