Job Description


Role: Hospital Complex Needs Practitioner
Location: Solihull
Salary: £26,544 - £28,640

Do you have an understanding of drug, alcohol, offending and health related issues, and experience of working with adults in an addiction setting?

I am currently working with a charitable organisation that is working with other organisations to be responsible for the delivery of drug, alcohol, homelessness, and gambling services, in the Borough of Solihull.


As a Hospital Complex Needs Practitioner, you will work with clients within UHB hospital primarily Heartlands/Solihull Hospital and various other community settings. You will be supporting clients in addressing their substance needs and liaising with medical professionals on the wards within the hospital and in the community. We are looking for someone who can complete both brief and psychosocial interventions and hold a caseload.


You need to be able to work independently and will be multi-agency working, working with HCA'S to Consultants daily. We are looking for someone who could deliver training in person and online and attend meetings and feedback in meetings associated with the hospital.


To be successful within this role we are looking for the following;
We are looking for an energetic and confident self-starter, you qualify for health/social care, youth and community work (e.g. NVQ Level 3 or above, DipSW, Mental Health Nursing, Counselling, Addiction Studies). Alternatively, we'll consider candidates with experience of working in the substance misuse and housing and homeless field with a commitment to complete NVQ Level 3 Health and Social Care.


It is preferred that you are a driver for this role due to the amount of community work involved.


As well as good knowledge of alcohol/drug and health-related issues, you're used to liaising with voluntary and statutory agencies and health professionals and comfortable engaging with clients in a variety of service delivery settings. What's more, you have a flexible approach, a commitment to the principles of confidentiality and excellent record keeping and Microsoft Office. You will need to be able to work flexibly in terms of working hours to support your clients.
