Housing Caseworker

Swansea

Full Time & Part Time hours available

£12.00 an hour

Are you passionate about supporting vulnerable adults who are needing support with their mental health, emotional well-being that could lead to issues maintaining a stable tenancy. We are recruiting on behalf of a leading housing charity in Swansea.

Role Overview:

Lead the assessment of individual's needs, gather additional information, and collaborate with them to create and review individual support plans and risk assessments. Work closely with organisations, local authorities and housing providers to ensure coordinated support. Liaise with agencies for long-term and crisis interventions, assist individuals in accessing information and advice, and provide resettlement support during moves. Maintain accurate records, and effectively share relevant information with colleagues using project communication methods.
