Young Persons Case Worker

Swansea

£12.49 an hour

Monday - Friday (with an on-call element on a bi-weekly rolling rota)

Are you passionate about making a difference in the lives of young people facing homelessness? Brook Street Social Care have an excellent 3-month opportunity with the possibility of an extension.

Key Responsibilities:

Manage a diverse caseload of young people experiencing mental health issues, substance dependency, and housing-related challenges.

Provide tailored support, advocacy, and guidance to help clients achieve stability and independence.

Collaborate with local agencies, healthcare providers, and housing services to coordinate comprehensive care plans.

Maintain accurate and timely records of all client interactions and progress.

Requirements:

Experience in social work, mental health, or a related field.

Strong understanding of issues related to homelessness, mental health, and substance dependency.

Excellent communication and interpersonal skills.

Ability to work effectively both independently and as part of a team.

Flexibility to participate in the on-call rota every other week.
